Boneset Herb (Eupatorium perfoliatum): The Classic Flu Herb

By Anjela Jeganathan – Holistic Medical Herbalist | Herba Naturalle

Boneset — Eupatorium perfoliatum — holds the distinction of being one of the most widely used herbs in American medical history. During the great influenza and typhoid epidemics of the 18th and 19th centuries, Boneset was the herb most commonly relied upon by both Native American healers and European settlers, and later by the Eclectic physicians who built the first systematic American herbal medicine tradition. Its common name derives not from its use in bone fractures but from the deep, aching pain in the bones and muscles characteristic of influenza — the “bone-break fever” — for which it provided the most reliable relief.

At Herba Naturalle, Boneset is listed in the comprehensive herb index as a primary acute immune and fever herb — one of the most specifically indicated plants for the influenza-type presentation.

Botanical Identity

Eupatorium perfoliatum is a perennial herb of the Asteraceae family, native to the moist meadows, swamps, and stream banks of eastern North America. It grows to 60–120cm, with distinctive opposite leaves that are joined at their bases and appear to surround the stem (the Latin perfoliatum — “through the leaf”). The white flower clusters appear in late summer. The aerial herb — leaves, stems, and flowers — is the medicinal part, harvested at peak flowering.

Active Compounds

  • Sesquiterpene lactones — eupafolin, eufoliatin, and related compounds; immune-modulating and anti-inflammatory
  • Polysaccharides — with significant immunostimulant activity
  • Flavonoids — quercetin, kaempferol, and eupatorin
  • Pyrrolizidine alkaloids (trace) — present in very small amounts; important safety consideration
  • Volatile oils — contributing mild antimicrobial and aromatic activity

Clinical Applications

Influenza and Viral Illness

Boneset’s most specific indication is influenza — the deep muscular and bone aching, high fever, sweating, profound weakness, and respiratory involvement of a proper flu. It is used in the acute phase to support the body’s own immune response, promote beneficial sweating (diaphoresis), reduce the severity of bone and muscle pain, and support recovery. It works with the body’s fever rather than suppressing it.

Immune Stimulation

The polysaccharide fraction of Boneset stimulates macrophage and natural killer cell activity — supporting the innate immune response in acute infections. The Smooth Muscle and Immune Reset Bundle addresses longer-term immune regulation.

Catarrhal Congestion

Boneset reduces mucous membrane congestion in the sinuses and upper respiratory tract — relevant in colds with significant nasal and sinus involvement. The Elderflower Complex provides complementary sinus support.

Fever Management

As a diaphoretic (sweat-promoting herb), Boneset supports the resolution of fevers through the natural process of perspiration — a physiologically supportive approach to fever that contrasts with pharmaceutical fever suppression.

How to Use

  • Tea (hot infusion — the traditional method): 2–3g dried herb steeped 10–15 minutes; drink hot, 3–4 cups daily during acute illness
  • Tincture (1:3): 2–4ml every 2–3 hours in acute illness; reducing to three times daily as symptoms improve

“Boneset is one of the herbs I reach for in acute influenza — its affinity for the deep bone aching and the need for a proper sweat to resolve the fever makes it almost specific for this presentation.”

⚠ Safety Considerations

  • Pyrrolizidine alkaloids: Trace amounts present — not suitable for long-term use; restrict to acute illness courses of 7–10 days
  • Pregnancy and breastfeeding: Contraindicated
  • Liver disease: Avoid due to pyrrolizidine alkaloid content
  • Large doses: May cause nausea and vomiting — use at therapeutic doses
